In recent years, urban areas, particularly old downtowns, have witnessed a surge in vacant houses, highlighting the need for measures to prevent their persistence and promote reuse. This qualitative study, conducted within the framework of Grounded Theory, focused on Seo-gu Ami-dong and Chojang-dong in Busan, representative areas of declining downtowns. The aim was to identify specific factors, based on the perspectives of owners and residents, that are associated with the occurrence, persistence, and reuse of vacant houses. Data were collected through in-depth interviews with residents and analyzed using coding and categorization techniques. The findings revealed that the occurrence of vacant houses was influenced by youth outflow due to local decline and decreased housing demand caused by an aging population. Factors contributing to the persistence of this phenomenon included local decline, owner indifference due to non-local residency, failure to manage unauthorized housing, owners’ economic vulnerability, and neglect of housing bought for investment purposes. Local residents’ desire to maintain community and affordable housing demand among vulnerable populations also led to vacant house reuse. This study underscores the need for measures to address vacant houses in areas with vulnerable elderly populations and provides foundational insights for practical solutions to address persistent housing vacancy in major cities’ old downtown areas that are plagued by unauthorized housing.
